• I understand that therapy is an investment.

    Intake sessions are billed at $200.

    Individual sessions are billed at $ 150.

    Therapy sessions are 55 minutes.

  • Before your first full length session you will be asked for a credit card to keep on file in our secure payment system. We accept all major credit and debit cards as well as HSA and FSA cards. At the end of each session your card will be charged. You can also pay with cash or check and a receipt will be given.

  • Yes, I am in Network with Commercial MVP, CDPHP and Highmark.

    If you are not a member of one of these companies, I am happy to provide you with a receipt (also known as a Super Bill) at the end of each month for out of network benefits.

    If you plan on using these benefits, I strongly recommend that you contact your insurance company to make sure they cover out of network services and to ask how to submit for reimbursement.

  • I ask that you cancel 24 hours in advance. You will be responsible for a payment of $75.00 if you provide less than 24 hours notice for a cancellation. This is necessary because the time is reserved and held exclusively for you.

  • Legal Services

    Please note that I do not provide court evaluations or court testimony and this is not a Forensic Social work office. If I am ordered to testify in proceedings, it seriously undermines my therapeutic practitioner-patient relationship, is very disruptive to the office routine, and is unfair to my other patients. Please inform me immediately if you are involved in, or plan to go to court. I accept clients only with the specific agreement that that they will not involve Melissa Anderson, LCSW, PLLC in any legal matters, including but not limited to child custody, workers’ compensation claims, and criminal cases. I do not provide legal opinions, do evaluation or testify for disability or child custody cases. I do not provide any documentation for emotional support animals (ESA).

    Verbal and physical aggression at any time in the office is prohibited and will result in immediate discharge.
